Community Pharmacy Access to the GM Care Record

Health Innovation Manchester is supporting community pharmacists in gaining access to GM Care Record via their IT system to boost the quality and safety of prescribing in the community.

Accessible via PharmOutcomes, the GM Care Record provides pharmacists and patients with the following benefits:

  • Improved patient care: pharmacists can make more informed decisions about prescribed medications
  • Improved medication safety: pharmacists can identify potential drug interactions and contraindications, minimising risk of adverse drug reactions

  • Increased communication: Continuity of care is improved through improved contact with health and care teams involved in a patient’s care
  • Accessing data securely and confidentiality: The GM Care Record is protected from unauthorised access and complies with all relevant laws and regulations.

How to gain access to the GM Care Record

Complete the 3 steps below to gain access to the GM Care Record

Step 1

Supply the IP address for each pharmacy to ensure that each pharmacy’s IP address is not blacklisted for access.

If you are unsure of your IP address, you can find it in Windows:

  • Select Start > Settings > Network & internet > Wi-Fi and then select the Wi-Fi network you’re connected to.
  • Under Properties, look for your IP address listed next to the IPv4 address.

Step 2

Ask your Data Protection Officer to complete the onboarding form to comply with data management requirements.

Step 3

Your Data Protection Officer will also need to download and sign the Data Sharing Agreement, then email to our Information Governance team at IG@healthinnovationmanchester.com.

The GM Care Record provides secure access to patient health information, including medication history, allergies, and care plans. This data enables pharmacists to offer safer and more personalised care.

You can access the GMCR via single sign-on (SSO) through PharmOutcomes system.

Pharmacists can view coded data, such as current medications, allergies, immunisation records, and recent diagnostic results. Access is restricted to coded information only—free text notes or consultation records are not available.

Patient consent for accessing records is already built into the GM Care Record. However, patients have the right to opt-out of data sharing for purposes beyond their direct care. Find out more here Your Privacy – GM Care Record.

Access to the GMCR is fully auditable, ensuring any data breaches or unauthorised access can be identified. Pharmacists must follow strict GDPR regulations and comply with professional standards set by the General Pharmaceutical Council (GPhC) to ensure data protection and confidentiality.

Access to the GMCR enhances medication management, prevents adverse drug interactions, and supports personalised care. It allows pharmacists to ensure safer dispensing by having a fuller picture of a patient’s health, particularly in emergency or high-risk situations.

Pharmacists receive specific training on using the GMCR, including data security, patient confidentiality, and the effective use of patient data within the GM Care Record to ensure compliance with GDPR and GPhC standards. To find out more, visit gmwearebettertogether.com/training.

If you experience any problems accessing the GMCR via SSO or need further guidance, please contact your pharmacy’s technical support or the GMCR support team.

First-Line Support: PharmOutcomes will provide their standard IT support directly to community pharmacies. This includes handling initial troubleshooting, access issues, and general inquiries related to the GMCR.  These will be logged through the PharmaOutcomes Help page.

Second-Line Support: GM IT Services will offer secondary support if the issue cannot be resolved by PharmOutcomes. This support is provided in accordance with a separate SOP that outlines the responsibilities and processes between GM IT Services and PharmOutcomes.

Third-Line Support: For complex technical issues that require further escalation, Graphnet will provide third-line support to GM IT Services. This level of support is governed by a separate SOP that details the interaction between GM IT Services and Graphnet.
