The GM Care Record

The GM Care Record is Greater Manchester’s shared care record, bringing together information from NHS organisations, local authorities and other care providers into a single, secure view.

Launched in 2020, the GM Care Record helps health and care professionals access the right information at the right time, supporting safer, more joined-up care for the 3 million people who live in Greater Manchester. Today it is one of the most widely used shared care records in England and a critical digital asset supporting integrated care, innovation and service transformation across the city-region.

Supporting the NHS 10 year plan

The NHS 10 Year Plan sets out three shifts for the future of health and care. The GM Care Record is already helping Greater Manchester deliver each of these ambitions:

Hospital to community

The GM Care Record allows all health and care professionals across Greater Manchester to access a shared view of information, helping people receive more joined-up care closer to home.

Analogue to digital

By connecting information from multiple systems, the GM Care Record reduces reliance on phone calls, emails and paper-based processes, enabling faster and more informed decision-making.

Treatment to prevention

The GM Care Record supports earlier intervention, personalised care planning and better understanding of population health needs, helping services move from reacting to illness towards preventing it.

Integrated care planning

Integrated care planning in the GM Care Record is creating a more connected approach that places patients at the centre of their care journey.

Sharing care plans with everyone involved in a person’s care ensures better coordination, improved outcomes, and a more streamlined experience for both patients and care providers.

Community Pharmacy access to the GM Care Record

Access to the GM Care Record is helping community pharmacy play an increasingly important role in delivering neighbourhood health services across Greater Manchester.

By providing pharmacists with access to key patient information, the GM Care Record supports safer clinical decision-making, enables Independent Prescribing and helps pharmacy teams work more effectively with primary, community and hospital colleagues.

Together, this is supporting the shift towards more proactive, joined-up care closer to home.
