Greater Manchester

The GM Care Record

The GM Care Record provides health and social care professionals with vital information to inform safer decision-making about patient care and treatment for all 2.8m of us in Greater Manchester.

It shares important information about health and care including:

  • current health or care issues
  • medications
  • allergies
  • test results
  • care plans
  • social care or carer support

The GM Care Record pulls patient information from several important areas of health and care including:

  • primary care
  • community services
  • mental health services
  • social care
  • secondary care
  • specialist services e.g. North West Ambulance Service and The Christie

This means that patients won’t have to repeat their medical history to professionals in different organisations, care plans will be followed consistently, and clinicians will be better equipped to identify patterns and plan care more effectively to meet the patients’ needs.

The amount of data that the GM Care Record holds is increasing all the time, so that a combined record can inform decision making for safer care and treatment.

The GM Care Record is an extension of integrated digital care records (IDCR) already live in each locality of Greater Manchester (e.g. Manchester Care Record, Bolton Care Record).

However, it collates patient information from across Greater Manchester into one place, making it easily accessible for health and care workers to inform direct care from across geographies and organisations.

See below for a 5-minute staff training film outlining the features of the GM Care Record:

Please note: If you can’t find a particular piece of patient data in the GM Care Record, it might be available elsewhere – do not assume they have no medical history and that the patient doesn’t have a particular condition or a latest test result available.
