Best use of digital for Integrated Care Systems

Partners: Pumping Marvellous, Dementia United, Graphnet

Thousands of individuals now receive more personalised care via new digital care plans and patient-facing app, My GM Care. This initiative builds on the success of the GM Care Record, offering better-informed care and treatment for people with conditions like dementia and heart failure. The project’s key results include an impressive 21% increase in GM Care Record usage compared to 2022/23, with over 21,000 healthcare professionals accessing more than 270,000 patient records each month.

Best use of digital for Social Care

Partners: Bury Integrated Delivery Collaborative, Safe Steps

In collaboration with the Bury Integrated Delivery Collaborative, primary care, care homes, the local authority, and SafeSteps, a digital initiative, aimed at improving early intervention for deterioration and falls prevention in Bury care homes, was launched in 2023. The successful deployment of a digital deterioration tracker (RESTORE2 and RESTORE2 Mini) alongside a multifactorial falls prevention tool resulted in a 20% reduction in ambulance callouts across 37 care homes in Greater Manchester in January and March 2024 compared with the same period in 2023.
