Manchester & Trafford Community Data Feeds Now Live in the GM Care Record  

Clinicians in Manchester and Trafford can now access live community data feeds in the GM Care Record (GMCR). This means frontline staff can see essential information such as community care referrals and which community teams their patients belong to, helping to improve care coordination across the region.

With Manchester and Trafford joining the other eight boroughs, all 10 boroughs of Greater Manchester now have live community referral data in the GMCR. This ensures that, for example, wherever a GM resident visits their GP, their community care details are visible which supports safer, faster, and more joined-up care.

This achievement is the result of collaboration between Manchester University NHS Foundation Trust, Health Innovation Manchester, Manchester and Trafford Local Care Organisations, Graphnet, and NHS Greater Manchester. It marks a key step towards digital maturity and a better experience for patients and professionals alike.

Racheal Jackson-Newsham, Senior GMCR Product Manager at NHS Greater Manchester, said, “Bringing Manchester and Trafford’s community data feeds live in the GM Care Record is a huge achievement. Now, every clinician across Greater Manchester can see the full picture of their patients’ community care. Together, we’re building a stronger, more connected health and care system for everyone.”
